It depends what the user is willing to store on their side.
For example if you create a cookie on their browser, you can issue a unique ID. Theoretically you could make a unique ID that could be self confirming based on IP address too, like kind of how credit card numbers have a built in self check. That would prevent it being duplicated or from wasting resources checking bad IDs. You could make this ID presented on handshake when user connects or requests data, automatically good for a week, updated every subsequent day they use the site.
Shit would go away if they dont allow that on their browser settings though.
You could also whitelist IPs at different levels. They pass the test they get white list level 1, you see them using the site legitimately for a day or two you give them level 2, etc.
None of this works if you're sharing an IP (for example using a VPN).
Realistically we dont punish ISPs and Telecoms enough.
Its absolutely possible to manage spam calls and bad actors on their side.

DDOS mitigation uses the same infrastructure you would use to fake a stage attack.
Think about that for a second.
All of the services that can use their datacenter full of computers to run their service and also launch DDOS attacks against some random server hosted on a mac mini stand to gain some benefit from Cloudflare gaining more of a foothold including the datacenters themselves.
